Symptom narrative

Resident reported on 10/15/25 that he began having some tingling & numbness in his left arm a few days after receiving the COVID vaccine but did not report it as he thought it would get better. When it became more painful & progressively worse he had difficulty performing some self care and reported it to Infection Preventionist. MD was made aware and ordered Gabapentin for the Neuropathy on 10/17 with increased dose ordered on 10/24/25 which was discontinued on 10/29/25 due to increased swelling in left arm and decreased ability to perform self care

Hypertension, Late effects CVA w/Right hemiparesis, Type 2 Diabetes, Hyperlipidemia

Simvastatin, Amiloride HCTZ, Clonidine, Clopidogrel Bisulfate, Ferrous Gluconate, Lisinopril, Metformin, Multivitamin, Carvedilol, Magnesium Sulfate
