Symptom narrative

Patient presented to vaccination clinic for high dose (HD) flu and covid vaccine 12+. While attempting to administer HD flu, the white vaccine fluid leaked from luer lock. Needle removed from patient's arm as fluid leaked out. Spoke with MD on site who agreed ok to give a new dose. FYI sent to PCP via chart. Reviewed with patient and daughter who agreed to receive HD flu as a third poke. Advised patient and daughter to call with any concerns, as there are nurses available 24/7.
