Symptom narrative

patient came to the pharmacy today to report "she experienced tingling on the face and tongue on the drive home and itchy eyes on 10/18/25 but it clears up by the next day. She did not take anything for the reactions. She did not contact us nor her primary care physician; however, she came by the pharmacy today to get the information needed to report to the vaccine adverse event reporting system (VAERS). She think that b/c it is the FLUZONE HD that why it cause the rxn but she not truly allergic to flu-shot. STC showed she had a Fluzone HD 9/15/20 @ her provider.)
