Symptom narrative

Teacher and parent notified with consent of child needed for vaccines day prior. Notified would get child Friday 10/31 just prior to Pre K naps. Front desk went to room Friday to get child. Name said with consent but para sent wrong child with similar name to clinic. In clinic child is 4 and spoke her name which is only one letter off from intended child. Mistake noted when child returned to class. Child verified again. FNP called mother of child who received vaccines. Situation was explained. Mother was understanding. Instructions given to monitor for mild fever like after any immunization received. Supervisor notified of incident and proper reports filled out. Documentation completed and submitted to proper contacts.
