Symptom narrative

patient came in for flu shot and ended up getting shingrix also. Patient had alzheimer's and an active case of shingles at the time that was not visible. Husband did not put that information on the patient consent form questionnaire and it was not mentioned when the form was gone over. The husband left a review later on complaining about the fact his wife got the dose of shingrix and mentioned she had an active case of shingles. When I spoke with him he stated that she seemed to be having more symptoms than normal but was doing okay. I advised to see a doctor if it worsened or did not improve after a few days.
