Symptom narrative

Child & Mother were scheduled for a Flu vaccine. Mother asked if they could get the Covid-19 vaccine added on. I administered the mother's vaccines first & then with the help of the mother, quickly gave the child her 2 vaccines. I documented the moms' vaccines & then the child's but had an issue documenting the child's Covid vaccine that I had administered. I realized her age was 8 years old and had given her the Adult Covid-19 dose. Mom was told of the action & was advised to report any adverse reactions.
