Symptom narrative

Patient age 62 was given FLUAD vaccine for age 65 years +. Verbal order for Fluad given to nurse. 2nd check was given to vaccine. Nurse administered Fluad high dose flu shot. during documentation of flu vaccine it was noticed that the wrong vaccine was administered. Provider notified and manager notified. pt was notified and pt was monitored for 15 mins and no reactions occurred
