Site: Pain at Injection Site-Severe, Systemic: Body Aches Generalized-Severe, Systemic: OBC made to patient 5 times left message once unable to reach. pt did leave a message stating she had a severe reaction to the vaccine that was administered and wanted to know which vaccine she received. she left message thursday 10/23/25 10:33 am-Severe, Additional Details: I reached out to patient to find out what her symptoms were and obtain additional information. I called several times yesterday and this morning, but was unable to reach her. Her number screens calls, but I followed the prompts and left a message once but there was no call back and I was unable to reach her. She left a message stating she had a vaccine on Tuesday 10/21/25 and had a severe reaction to it and wanted to know what the name of it was. I'm not sure if benadryl/epi given
