Systemic: no effects-Mild, Additional Details: pt came to pharmacy wanting a flu shot plus she gave us 3 papers that md wants her to get. one of which is pneumonia. had her filled out paperwork and before we gave her vaccine, verified her DOB plus what we are given her. after flu was given, she said she already had flu at the md's office. there was a miscommunication on both party. apologize to pt and called md to inform of situation. pt is ok. no need to contact
