Systemic: Patient did not report any adverse reactions-Mild, Additional Details: Patient was administered Spikevax vaccine under registration of wrong patient with same first name and last name (different DOB). Patient's wife reported on 10/25/2025 by identifying wrong DOB on the vaccine record. Patient who was administered vaccine did not report any adverse reactions from the vaccine.
