Symptom narrative

Site: Bruising at Injection Site-Mild, Site: Pain at Injection Site-Medium, Additional Details: The needle was defective and did not attach to syringe properly and liquid poured out when syringe was plunged. No vaccine was administered so another injection was given. Patient called the next day because arm was very sore. He reported no swelling or redness but small dime sized bruise
