Symptom narrative

Site: Itching at Injection Site-Mild, Site: Redness at Injection Site-Medium, Additional Details: Pt states that she scratched the injection site after removing the bandage. The swollen came the next day. The redness and swollen reduce got better today. She complaint when she cough it feels pain. RPH recommended pt to see doctor regarding the reaction. Pt states she will monitor in the next couple day. If it get worse, she will make an appointment.
