Symptom narrative

3 vaccines were administered to pt (HPV/MENQ/TDAP). HPV and TDAP were administered on left arm IM, then last vaccine MENQ was administered successfully to right arm IM. Shortly after placing Band-Aid pt fainted and fell from table where she was sitting. STAT was called provider was in the room while vitals were taken-BP, pulse and oxygen was checked and stable. Placed ice pack to pts forehead, lifted legs and held her on the floor for a bit until stable enough to transfer to lay on table bed and observe. Information on fainting was provided to pts mother. Pt was observed then discharged after
