Symptom narrative

Series had been completed in another state. Patient was assessed and this was not realized during the consult. Inadvertently gave a Hepatitis A; during the appointment and patient received Hepatis A series although had one already on 7/162019, and 1/16/2020. I did call the patient to explain this occurred as well.
