My child was administered the wrong vaccine. I had to bring it to the nurse's attention that it was the wrong vaccine. She was apologetic. I called the next business day and spoke with the office manager, also requesting my child's vaccination forms. The office manager was unaware of the incident, and the vaccine was not reported as an error or at all on her vaccine form. The office manager did not encourage to report the issue either. Although the vaccine was not harmful to my child, I do not appreciate how the situation was handled. I do not wish for anything bad to happen. However, this form of negligence must be reported.
