Symptom narrative

Please note: The pt was seen out of state at the [name withheld] clinic and not at our health department for the vaccinations. Therefore, we have no records of the vaccinations with lot numbers available, just notes from the ARNP that saw the pt from the [withheld] County Clinic and listing of vaccinations in provider documentation. The ARNP called the County Health Department to report that the pt was seen by him on 11/3/25. Pt c/o 102.8 °F temperature, grunting, shivering, lips turning blue six days after vaccination on 10/27/2025. MOC was concerned about a delayed vaccination reaction. Pt stated from clinical notes that a fever did occur on 10/26/2025 and resolved the next day. MOC brought pt to the County Clinic on 11/03/2025 with the concerns of grunting, shivering, lips turning blue instead of returning to [withheld name] Clinic.
