Symptom narrative

I gave patient her flu vaccine. After administering vaccine patient says she did not feel well. She looked pale. Patient said her vision went blurry, ears ringing and she got hot. I assisted her up on the bed and took her BP. BP measurement was 64/33, HR was 49, O2 was 98%. I then let my supervisor RN and got Dr. I then got her a cold pack because she was feeling warm. Along with a ice pop and chips per Dr.
