I provided the incorrect vaccines: Pentacel and PCV20 which are the patients 4 month vaccines before the vaccines were due. Patient is 2 months and 21 days and did not need the following vaccines listed until patient is 4 months of age. I did inform the patients provider and also the doctor in clinic at the time. I also informed the patients mother I gave the incorrect vaccines listed. The patients parent voiced understanding. Per Dr's instruction, advised mom if any signs of persistent fever, fussiness, behavioral changes, redness at the site, or swelling at the site to contact the clinic immediately or take pt to the nearest ER. Patient was monitored for fifteen minutes prior to leaving and did not have the associated symptoms. Patient was alert and oriented. Patient was also fine at home that evening per the patients mother.
