Symptom narrative

The Influenza vaccine was administered as part of our school-based mass vaccination clinics. The child's legal guardian had signed consent for the child to receive the influenza vaccination at school on 10/16/25. The vaccine was administered at school on 10/16/25 by LPN (County Department of Public Health). When we went into the Immunization Registry to enter the immunization, the system would not allow us to enter the vaccine, stating the the child had received an influenza vaccine on 10/10/25. I contacted the child's guardian to discuss this. She reported that the child was seen to receive her COVID-19 vaccine. I advised the guardian to contact facility to verify what the child had received. Facility verified that she received influenza at her visit. The child's guardian did not understand how this had happened, as she expressed to me that she explicitly requested the COVID-19 vaccine at the child's appointment. We discussed that the child had now received 2 doses of influenza and no doses of COVID-19. I educated her to monitor for signs and symptoms of adverse reactions and what steps to take should this occur. Guardian stated understanding. No adverse outcomes were reported by the guardian as of this date.
