Symptom narrative

Patient's ex-Husband reported that Pt received 2 vaccines in her left arm. Experienced extreme pain and decrease in motion of left arm. Was seen in urgent care 2 days after shot and prescribed meloxicam for the pain. Meloxicam did not take pain away. Pt was then seen by healthcare professional and received a cortisone shot in left shoulder.
