The influenza vaccine was administered as part of our school-based mass vaccination clinics. The child?s legal guardian had provided written consent for the child to receive the influenza vaccination at school on 10/16/25. The vaccine was administered on 10/16/25 by RN of the County Department of Public Health. Upon attempting to enter the immunization into the Immunization Registry, the system indicated that the child had already received an influenza vaccine on 10/15/25. I contacted the child?s guardian to clarify this discrepancy. The guardian reported that the child had attended facility to receive a COVID-19 vaccine. I advised the guardian to contact facility for verification. Facility confirmed that the child had, in fact, received an influenza vaccine during that visit. The guardian expressed surprise, noting that she had explicitly requested the COVID-19 vaccine. We discussed that the child had now received two doses of influenza vaccine and no doses of COVID-19 vaccine. I provided education to the guardian on monitoring for signs and symptoms of adverse reactions and appropriate steps to take if they occurred. The guardian indicated understanding. No adverse outcomes have been reported as of this date.
