Symptom narrative

Returned for Ixiaro #2 and Vaxchora. Vaccinated against COVID (Moderna), Tdap, and Hep B on the morning of 11/3/25. Vaxchora given without acute distress. Ixiaro administered at 2:09 PM - reported warmth in L deltoid, radiating to the hand. Swelling appeared below the fingers at 2:10 PM with slight itching. Diphenhydramine 50 mg IM administered at 2:12 PM. Client declined EMS - swelling, warmth, and itching reduced in the 5 minutes following. NAD after 30 minutes.

Currently takes Vitamin D2 1.25mg weekly, spironolactone 150 mg daily, metformin 500 mg BID, and Lexapro 10 mg BID.
