Product administered at inappropriate site

Symptom narrative

Both vaccines reported to be given too low for correct administration site. Stated nurse did not utilize landmarks. Today, I assessed the site. Vaccine was given low, but upon assessment was within the deltoid muscle for both vaccines. Education given to administration nurse. Patient was educated on signs and symptoms of any concerning issues as well as given information to report back to us if any concerns arose.
