No additional AE; PROQUAD was involved in a temperature excursion on 10/7/2025 and one dose was administered to a patient on 10/15/2025; This spontaneous report was received from the Office Manager and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 15-Oct-2025, the patient was vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) solution for injection (lot #Z008868, expiration date: 10-Nov-2026), 0.5 ml (two dose series), administered by unknown route as prophylaxis. The vaccine was reconstituted with sterile diluent (lot # and expiration date were not provided). The suspect vaccine was administered after being improperly stored (temperature excursion on 07-Oct-2025) (Product storage error). No additional adverse event.
