Symptom narrative

Reported Symptoms: Narrative: After properly reconstituting the antigen with the manufacturer provided diluent syringe attached to the manufacturer supplied luer-lock vial adapter the nurse retrieved a needle from the clinic's supplies attached a 23Gx1" needle (Magellan 23Gx1" Lot number 240257 expiration 02-28-2029). The needle was properly secured to the diluent syringe. After administration in the patient's arm the nurse went to withdraw the syringe and needle but the needle became detached from the syringe and remained in patient's arm. This is the 2nd such event at this clinic with this Lot of RSV. The concern is the female luer-lock hub on the manufacturer provided syringe is either faulty out of the package or made faulty during the process of luer-locking the syringe to the provided vial adapter. Since this event, we've switched lots at this clinic and are no long experiencing this problem. Other Relevant HX: Other: Other
