Symptom narrative

10/31/2025 low grade fever, body aches 1900, 11/1/2025 0600A fever 101.6, headache, body aches, fatigue, R stomach discomfort. Fever increased to 102F most of the day with chills. Took OTC naproxen 220 mg mid-day. Took acetaminophen 250 mg in the evening and fever broke at 2300. 11/2/2025 fever returned at 0200 and took more acetaminophen 250 mg and fever down at 0400A. Had low grade fever, chills, headache, fatigue R stomach discomfort/pain , nausea. 11/3/2025 Woke feeling okay and went to work and developed low grade fever 99.6F chills at 1000A. Took Tylenol 1000 mg at 1030A. Felt ill all day. Noticed itchy rash after shower on back, trunk, arms, legs at 2000 and went to bed. 11/4/2025 woke at 1AM with headache, rash spreading, fever 101F, R stomach pain, and went to [withheld] Memorial [withheld] ER.

11/4/2025 Elevated AST 489, ALT 597, ALP 501 and bilirubin 1.5. Elevated ESR 40, CRP 8.4, Ceruloplasmin 73. Negative Hepatitis panel. Elevated Procalcitonin 0.19 , ANA Positive, 1:160. MRI MRCP NORMAL ABDOMINAL MRI AND MRCP, CSF MENINGITIS PANEL NEGATIVE, MRI BRAIN - MILD SMOOTHDIFFUSE DURAL THICKENING AND ENHANCEMENT, NONSPECIFIC, ETIOLOGY INDETERMINATE. GIVEN PROVIDED HISTORY OF FEVER AND HEADACHE, INFECTIOUS ETIOLOGY, I.E MENINGITIS IS CONSIDERED. NO LEPTOMENINGEAL ENHANCEMENT OR EDEMA OBSERVED.
