Symptom narrative

"I called patient to discuss vaccinations. Identity was verified by name and DOB. Wife was on call as well. On review of chart High Dose Flu was given, when it should have been normal dose Flu vaccine due to him being younger than 65 years old. This could be causing worse side effects. Meningitis vaccine was the correct one per charting. Discussed with patient that this will still protect him from flu this season, but may cause a larger immune response. Current symptoms are: Light headed and different nausea from metformin, muscle spasms and headache and bone pain. Has gotten worse, but is managing with conservative care at home: took aleve and/or Motron as needed. Encouraged patient to drink plenty of water, continue NSAIDs PRN, and rest today. Expect symptoms to improve over the next 48 hours. If not please reach back out to me/on-call team."
