Symptom narrative

Staff pulled a Fluad 65+ for pt that is 64 yrs and a Prevnar 20. When staff scanned the Fluad it alerted her to pt only being 64. She then grabbed the Flulaval and all three were on her desk. She thought she grabbed the Fluad to put back in the refrigerator but she actually grabbed the Prevnar 20 and put it back where the Fluad goes. She then gave pt both a the Fluad and Flulaval influenza vaccines. She noticed later that the Prevnar was what she put back and told the pt and the provider. Provider said pt would be fine and to have him come back in two weeks to get the Prevnar 20.
