Symptom narrative

Patient reports hives appearing in body next day after receiving vaccinations. Also reports blisters in her nose, tingling in her toes (feels like a burning sensation on her legs). She reports left arm redness and soreness - arm is warm to the touch. Patient reports pain to the neck and tingling sensation that is felt on her legs is also being felt on her head. Patient states that a couple of days after these symptoms appeared, she fell outside and blacked out. She went to the doctor to be evaluated due to the continuing symptoms.
