Symptom narrative

Vertigo started approximately 4 days after each of her hepatitis B vaccines. This is the second occurrence, after the first two shots. Incident #1 Hep B and Hep A vaccines given together 9/17/2025, vertigo started 4 days later, no current or recent illness in the month prior to vaccine. She was treated during walk-in weekend hours at her PCP's office, for sinusitis. This was due to mild congestion that started after the vertigo started. Her symptoms resolved completely and she returned as scheduled for her second Hep B shot. Incident #2 Hep B and flu shot co-administered on 10/28/2025, vertigo started approximately 4 days later, no current illness at the time of the injections. No prior AR to any vaccines. Second vaccines given when Hep B second shot due, along with flu vaccine both given on 10/28/2025 Hep B (Engerix-B), GlaxoSmithKline Biologicals, lot # 9K34M, Intramuscular right arm, second in series Flu (Afluria), Seqirus, lot #AX5812A, intramuscular left arm, annual vaccine
