Symptom narrative

Patient is still in severe pain from 1 of the 2 shots. Patient has lost range of motion in his arm, numbness & tingling radiant down his arm to his elbow. He cannot lift his arm. lost range of motion and moderate paralysis of the arm. He has bolts of excruciating pain. He has to make adjustments to daily living trying not to further aggravate the arm/point of injection. He has difficulty sleeping. It impacts him when trying to put clothes on. The day is the shot he radiates chills though his body and went to bed at 8;45 when usually he is awake until 1-2am Patient is autistic avoiding human contact. his emotions are muted. So for patient to daily express pain from the injection is a big deal. I believe his auxiliary nerve in his upper arm was punctured when the shot was given. We need a follow up and diagnostic testing.

Medical history

Type 2 Diabetes, Hyperlipidemia, Asperger's Syndrome, OSA on CPAP

Other medications

Ability, Wellbutrin, Colchicine, Lisinopril, Modafinil, Crestor, Synjardy, Mounjaro

Lab data

Left arm weakness With pain that is persistent 1 week after vaccination administration. Will send referral to neurology for EMG and consultation, considered possible nerve injury. Sending multiple referrals to see who can see him first. Will also start referral to physical therapy to work on strengthening exercises. Advised icing area, okay to alternate with heat if that gives more comfort. He declines anything additional for pain at this time. Orders: ?
