1030- Noted patient annotated yes on history of passing out while donating blood on Flu vaccine screening sheet. Dr notified to come access patient prior to giving vaccine. 1040- Dr to clinic and completed his assessment clearing patient to receive Flu vaccine and signed screening form. 1045- Flu vaccine administered, and she was instructed to wait 15 minutes for possible reaction then nurse would bring her completed memo to give to her supervisor. 1100- Nurse was following up with patient and her completed memo and ask how she was feeling. She stated ? I was fine until you walked up? Nurse noted she was flushed in the face, and she started c/o N/V. Then she started having shaking and chills and c/o neck muscles being tight. 1105- After consulting with RN patient was transported to ER via wheelchair. LPN assisted with check in at ER and transferred care to ER nurse.
