Symptom narrative

Wife contacted pharmacy and said patient was not doing well after his vaccination. His arm was sore, is still sore, and he feels "yucky." Family feels his condition has went "down hill "since the vaccine, but wife said that may be a coincidence. Patient woke up and thought his house was on fire, but wife said he has had other delusional episodes in the past. Wife said patient did not report and adverse effects from receiving vaccination #1 and #2 of the TwinRx. I recommended that the family reach out to his provider or been seen in the ER for evaluation.
