On 27-OCT-2025, the patient received Priorix. On 27-OCT-2025, an unknown time after receiving Priorix, the patient experienced inappropriate preparation of medication (Verbatim: administered only the Adjuvant of Priorix to a patient without mixing it) and inappropriate dose of vaccine administered (Verbatim: Active ingredient not added to diluent). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
