Inappropriate Reconstitution Technique; Maladministration of two diluents of the SHINGRIX vaccine; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate preparation of medication in a 73-year-old female patient who received Herpes zoster (Shingrix) (batch number HM9N5, expiry date 25-MAR-2027) for prophylaxis. On 28-OCT-2025, the patient received Shingrix. On 28-OCT-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Inappropriate Reconstitution Technique) and inappropriate dose of vaccine administered (Verbatim: Maladministration of two diluents of the SHINGRIX vaccine). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 28-OCT-2025 The registered pharmacist called to ask about advice on a situation where two diluents of the Shingrix vaccine were administered the same day to a patient.
