Symptom narrative

A patient presented for the administration of the Pneumococcal, Rotavirus, and Vaxelis vaccines. During the vaccination process, the DTaP vaccine was inadvertently administered instead of the intended Pneumococcal vaccine. The error was identified shortly after the administration. The provider was immediately informed of the incident and will be contacting the patient directly to disclose the event, discuss the circumstances, and coordinate any necessary follow-up care or monitoring. The incident will be documented within the patient's medical record for proper tracking and future reference.
