Symptom narrative

It was discovered on 11/6/2025 that when the patient received this vaccine on 11/3/2025 it was administered intramuscularly instead of subcutaneously. At the time of the visit, and post administration of this vaccine, this patient did not experience any local or systemic side effects. The patient was contacted on the day of discovery (11/6/2025) and informed of the error in administration and how the vaccine should have been given. The patient was educated on local and systemic side effects and it was confirmed with patient that they have not experienced any side effects at this time. These local and systemic side effects that were reviewed with the patient include: local - Pain, redness, and itching at the injection site & systemic - possible fever, headache, tiredness, nausea, chills, and muscle aches. The patient was advised that the dose that was administered was a valid dose and does not need to repeat administration of this dose. It was also confirmed with pt that they should return for their scheduled follow up in clinic to receive their second dose and the patient verbalized understanding. The patient was lastly informed to call back his primary care physicians office if there are any questions or concerns regarding this administration of this vaccine.
