Symptom narrative

c/o severe aching pain to injection site shortly after receiving injection patient states, thought pain would go away, around 8pm noted pain was very severe, not able to lay on L-shoulder, difficult moving arm, reaching backward behind self, picking things up, lying on L-shoulder make pain worse.

Current illness

Denies any illness at time of vaccination and up to one month prior.

Medical history

Hypertension, Type II Diabetes Mellitus without complications, Hyperlipidemia, low back pain, carpal tunnel.
