Symptom narrative

patient received HPV vaccine after five minutes nurse heard patient fall. patient's dad reported patient fell forward off of the exam table. Patient was helped up by father and came to within a minute. was examined by physician with no injury noted. Patient given ice for face and water. continued with 15 minutes of observation with no other adverse events noted. Patient discharged home.
