Patient has a generalized maculopapular rash that started four days after he received Hib and Prevnar 20 vaccines in his left anterior thigh. He also received rotavirus vaccine at that time. He continues to have mild swelling and minimal discomfort in the leg. He is also having difficulty sleeping since the vaccines were given. He has been spitting up his formula more in the past week and is not sleeping well and eating more frequently overnight. He is taking 4 oz bottles of Kindemil. It is possible that he has abdominal upset from the rotavirus vaccine as well. Recommended to start pepcid for reflux. Once he has improvement of spitting up, would increase his kindemil to 5 oz bottles during the day to see if he needs only one at night. If he continues to have spitting up, would consider switching his formula to Alimentum or Nutramigen that has smaller proteins. Will report rash and stomach upset after Hib, Prevnar 20 and rotavirus vaccine to VAERS government vaccine website for adverse events. Chart updated to reflect rash with Hib and Prevnar 20 vaccines and spitting up after rotavirus vaccine. Would recommend to wait until rash is completely resolved for 2 weeks before giving him the Pediarix vaccine, which he is also due for.
