Symptom narrative

Parent did not disclose previous history of syncope with vaccination. Patient was given one of two injections ordered. Approximately 5 second after the injected patient began to sway on the patient bed. Patient was laid down immediately and he reported dizziness and tingling in his body. Vital signs were done and doctors was notified. due to down in bp and symptoms patient remained laying down for approx 20 minutes. After he denied any dizziness and bp returned to baseline. Doctor was notified and patient was cleared to go after walking around the room to ensure he was no longer dizzy.
