Symptom narrative

Patient came in for WCC Dad requested he receive the Covid and Flu vaccine. I mistakenly gave Patient a Pediatric Covid vaccine instead of an adult vaccine. After talking to CDC on 11/6/25 guidelines changed on 11/4/25 and patient should have the updated adult dose ASAP. I called parent who did not wish to have his son come back for the adult Covid and thought with getting the Flu vaccine also he would be fine for the year. I did tell him to let me know if he changes his mind.
