Symptom narrative

Underdose; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of accidental underdose in a 80-year-old female patient who received Herpes zoster (Shingrix) (batch number C9X3R, expiry date 05-JUN-2027) for prophylaxis. Previously administered products included Shingrix (received first dose on 15-AUG-2025). On 29-OCT-2025, the patient received the 2nd dose of Shingrix. On 29-OCT-2025, an unknown time after receiving Shingrix, the patient experienced accidental underdose (Verbatim: Underdose). The outcome of the accidental underdose was not applicable. Additional Information: GSK receipt date: 29-OCT-2025 The reporter reported that pharmacy technician was administering a Shingrix shot when the needle came out and smaller volume was administered to the patient. This was the second shot, first was administered back on 15th August 2025. The reporter wanted to know if revaccination was needed and when to do it.
