Symptom narrative

the dose came out of the patient's skin/muscle and the patient did not receive their full dose.; No additional AE reported.; This spontaneous report was received from a Medical Assistant and refers to a 6-month-old patient of unknown gender. The patient's concurrent conditions, medical history, previous drug reactions, drug allergies and concomitant therapies were not reported. On an unspecified date, the patient was vaccinated with a dose of Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAX HIB), 1 dosage form (route of administration, site of administration, lot #, and expiration date were not provided) as prophylaxis. However, the dose came out of the patient's skin/muscle and the patient did not receive their full dose (Incorrect dose administered). The caller said the medical assistant who administered the vaccine said it was only a little bit of the vaccine; however, the parent who witnessed the incident said it seemed like it was all of the vaccine that came back out of the injection site. No additional AE reported.
