Symptom narrative

Patient received the HPV vaccine and then had a blood draw. SYNCOPAL EPISODE WITH BLOOD DRAW- PASSED OUT FOR APPROX 30 SECONDS. ICEPACKS APPLIED, JUICE ADMINISTERED, HOWEVER, AFTER EPISODE, VOMITED. LOOKED BETTER AND TRIED TO GO HOME AFTER RESTING FOR 15 MINUTES AND HAVING STABLE VITALS. FELT NAUSEOUS AND FAINT AGAIN AND WAS PLACED PT BACK IN A ROOM WITH ICEPACKS. VITALS MONITORED, ZOFRAN GIVEN AND PATIENT TOLERATED SUCKING CANDIES. MOM CAME TO ASSIST AND PATIENT LOOKED WELL AFTER RESTING FOR APPROX 45 MINUTES IN THE OFFICE
