Symptom narrative

Wrong vaccine; Wrong age; This non-serious case was reported by a physician via call center representative and described the occurrence of wrong vaccine administered in a 18-month-old male patient who received DTPa (Reduced antigen) (Boostrix) for prophylaxis. Co-suspect products included DTPa (Infanrix) for prophylaxis. Previously administered products included Pediarix (1st dose received on 18-APR-2024), Pediarix (2nd dose received on 10-JUL-2024) and Pediarix (3rd dose received on 8-JAN-2025). On 06-AUG-2025, the patient received Boostrix. On an unknown date, the patient did not receive Infanrix. On 06-AUG-2025, an unknown time after receiving Boostrix, the patient experienced wrong vaccine administered (Verbatim: Wrong vaccine) and inappropriate age at vaccine administration (Verbatim: Wrong age). The outcome of the wrong vaccine administered and inappropriate age at vaccine administration were not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK receipt date:28-OCT-2025 The Physician mentioned that she accidentally administered Boostrix instead of Infanrix to an 18 month old patient on 6th August 2025. Patient got 3 prior Pediarix doses on 18th April 2024, 10th July 2024 and 8th January 2025. The reporter consented to follow up.
