Symptom narrative

Administration of only the diluent; Administration of only the diluent; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 62-year-old female patient who received RSVPreF3 adjuvanted (Arexvy) (batch number 24M94) for prophylaxis. On 28-OCT-2025, the patient received Arexvy. On 28-OCT-2025, an unknown time after receiving Arexvy, the patient experienced inappropriate preparation of medication (Verbatim: Administration of only the diluent) and inappropriate dose of vaccine administered (Verbatim: Administration of only the diluent). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date: 31-OCT-2025 The reporter was doing inventory, and noticed that an Arexvy vaccine, there was a missing diluent, but was still had the powder in there. They 2 patients that received the vaccine, so the reporter assumed that only one of the patients got the diluent.
