Within 24 hours after receiving an influenza vaccination in the right arm, the patient developed sudden severe shoulder pain in that arm. Over the next several days the pain spread across the upper back and into the left shoulder. The right-arm pain later improved somewhat, but the left arm developed progressive weakness and pain. At present (2 months post-vaccination) the patient cannot lift the left arm above shoulder height because of marked weakness in the deltoid and surrounding muscles. No fever, rash, or systemic illness occurred. Evaluation for possible post-vaccination brachial neuritis (Parsonage Turner syndrome) versus cervical radiculopathy is in progress. The event has caused major limitation of daily activities and concern for neurologic injury.
