Systemic: Allergic: Difficulty Swallowing, Throat Tightness-Severe, Systemic: Allergic: Itch Generalized-Severe, Systemic: Chills-Medium, Systemic: Fever-Medium, Additional Details: Runny nose and coughing for a month and the patient was unsure of the date of immunization but stated it was 3 years ago the first week of October. She used OTC for some symptoms like ibuprofen, Tylenol, NyQuil and [withheld] Medical Center prescribed a cough syrup that had codeine in it.
