Symptom narrative

Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Systemic: Vomiting-Mild, Additional Details: Patient with a history of fainting with vaccines temporarily lost consciousness. He was brought to the floor until he came to. Patient was given sips of water. He then tensed up which caused him to temporarily pass out again. His vitals were checked and normal. He vomited up water from the anxiety of the situation. An ice pack was placed to the back of his neck. He rested with his parents for 15 minutes until feeling ok to leave.
